Prior to commencing the Level 4 Professional Practice or Level 4 Preparing for Leadership and Management in Health and Social Care, you are required to undertake a number of written tasks and provide documents to support your progression on to the programme.

 

Task 1 - Personal statement (PS)

Write a typed Personal Statement which outlines your career path, how this qualification fits into your career development plan and to include the identification of any support needs. Please read the guidance notes to support you to write your personal statement. 

 

Task 2 - Curriculum Vitae (CV) 

Please provide a current and up to date CV. 

 

Task 3 - Job description (JD)

Please provide a current and up to date job description. 

 

Task 4 - Answer the set questions below:

  1. What do you understand about the Equality Act 2010?

  2. What does the Code of Practice for Social Care Workers set out to do?

  3. What is your understanding of person-centred planning?

 

Task 5 - Assignment

Please complete a 500 - word assignment on one of the following topics:

  1. The Social Services and Well-being (Wales) Act 2014 - Please describe how you ensure the 5 principles of the Act are followed in your role.

  2. The Mental Capacity Act 2005 - Please describe how you implement the Mental Capacity Act in your setting.

  3. Code of Professional Practice for Social Care Workers - Please describe how the Code sets out the standards of behaviour and practice required by workers in the health and social care profession.

  4. Safeguarding Awareness - Please describe how you take responsibility for protecting the health, well-being, and human rights of people in your care who might be vulnerable to abuse.

  5. The Social Care Manager - Please describe how you ensure that you provide high quality care and support in your leadership role in a care setting.

 

Complete the assignment using academic writing skills – correct use of grammar, spelling, punctuation, and paragraphing etc.

Remember to use double spacing and use font size 12 as a minimum.

 

On completion of the written tasks, your answers will be assessed to judge your suitability to proceed onto the UPK for the first mandatory unit of the learning programme.
